Introduction

 

This questionnaire has been designed to obtain feedback from you regarding your experiences of change in the organisation and to highlight areas that are performing well or need improvement.

It has been divided into 3 sections:

i) information on your role and position within the organisation including implementation of change

ii) reflecting on your experience of change in your organisation (views vary from ‘strongly disagree’ to ‘strongly agree’)

iii) ranking of support mechanisms
